Application Procedure

A Kates grant request form of questions (enclosed/requested) must be submitted  for formal consideration.  If an agency wants to discuss a grant idea with the foundation before proceeding with the grant request form they may do so.  

When submitting a grant request the following documentation is required:

  1. A current copy of the letter of exemption from Federal Income Taxes under 501(c)(3) of the Internal Revenue Code.
  2. Certification that the applicant is not a private foundation under section 509(a) of the Internal Revenue Code.
  3. A copy of the Internal revenue Service form 990 for the most recent year.
  4. If the applicant is a unit of government, evidence of this.
  5. The latest income statement and balance sheet as audited by a certified public accountant.
  6. The name and qualifications of the individual requesting and administering the grant.
  7. A list of the Board of Directors, their addresses and occupations.
  8. Current annual agency budget with year to date actual  revenue and expenses.
  9. A Kates grant request form of questions (enclosed/requested) must be submitted through the postal service for formal consideration .
  10. An agency may only make one request per year.  

Any and all supporting materials such as printed publications that describe the organization that may be useful. From the information provided the foundation’s board of Directors and review committee will determine if the request is compatible with the foundation’s objectives. An onsite visit to the agency may be required.
